About the Role
DBHRS Ltd is looking for experienced Assessment and Enablement Officers to support a Local Authority Adult Social Care service on a temporary basis. You will review care and support arrangements, work with residents and partners, and make clear recommendations that promote independence, wellbeing and positive outcomes.
Key Responsibilities
- Review Adult Social Care support arrangements.
- Check that care packages meet eligible needs and outcomes.
- Use strengths-based, person-centred practice.
- Assess changes in needs, risks, circumstances and wellbeing.
- Work with residents, families, advocates, providers and partner agencies.
- Apply the Care Act 2014, Mental Capacity Act 2005 and safeguarding principles.
- Challenge support arrangements where better options may be available.
- Produce clear review documentation, assessments and recommendations.
- Identify themes and opportunities for service improvement.
About You
- Experience working with adults with learning disabilities, mental health needs and physical disabilities.
- Experience reviewing Adult Social Care packages.
- Good knowledge of the Care Act, Mental Capacity Act, safeguarding and strengths-based practice.
- Experience across supported living, home care, residential care or community services.
- Confident completing assessments, reviews, risk assessments and support plans.
- Strong professional judgement and evidence-based decision-making.
- Excellent communication and relationship-building skills.
- Well organised, with the ability to manage a varied workload.
- Committed to independence, wellbeing, choice and control.
