Description
We are in search of a Hospital Social Worker or Care Manager. To assess the needs of individuals with complex needs, taking into account levels of risk and their strengths, to protect their rights and promote their independence and interests. The successful candidate will play a vital role in delivering services that promote independence, safeguard vulnerable adults, and support the well-being of our community.
Responsibility
To offer professional guidance to people and their carers to enable them to make informed choices about the care services that they receive and to produce a person-centred support plan which promotes their independence, dignity, choice, self-esteem and well-being.
Qualification – Essential
- GCSE Grade (or equivalent) in English Language and Mathematics.
- Degree in Social Work) or a recognised Occupational Therapy qualification,
- or a relevant community Nursing qualification.
- Social Work England (SWE) Number
- Microsoft Office, Word, Excel, Outlook, social media and smartphone.
Essential - Skills, Abilities, & Experience
- Experience working in hospitals or adult social care.
- Strong knowledge of the Care Act, Mental Capacity Act, and safeguarding procedures.
- Excellent assessment, communication, and multi-agency working skills.
- At least 2 years’ experience in a Local Authority, Health or voluntary sector setting.
- Knowledge of relevant legislation and a fundamental understanding of, and commitment to, the principles of personalisation, independence and choice.
- Experience of working with older or physically disabled people and knowledge of service provisions for these groups.
- Experience of working in a multi-disciplinary way.
- Experience with assessment work.
- An understanding or experience of budgets and financial controls.
- Ability to carry out accurate and timely assessments and promote self-directed care.
- An ability to communicate clearly in writing, verbally and through reports with service users, carers and other professionals.
- Ability to work independently using own initiative, prioritise workload, and meet deadlines.
- Ability to negotiate on behalf of the Authority and/or the service user, with other agencies.
- Ability to work proactively, to pre-empt / and or prevent and manage a crisis.
- Ability to work as a team member in a multi-disciplinary service.
- Ability to keep comprehensive case records, which can be shared with service users in line with the Open Files Policy.
- Ability to undertake a comprehensive assessment of service users’ support needs.
- Ability to communicate professionally in a language other than English (desirable
